If you've filled a prescription for any of the 100+ medications currently covered by innoviCares, and paid out of pocket for your medication, you can submit a claim to be reimbursed.
To submit your claim
- Get started by entering the Client ID located on your innoviCares card in the form below.
- Complete the online form and upload a photo of your prescription receipt, or fax or mail a copy of your receipt to STI Technologies Limited.
Please note, reimbursement is only available on STI Technologies Limited
prescription cards, including the innoviCares card.
